Friends and Family Test 

The Friends and Family Test Questionnaire is a very useful tool that we use to collect feedback about the service we provide.

We want everyone to get the best possible care and treatment and The Friends and Family Test will help us do that so we can identify what we are doing well and what we need to build on. 

You can tell us if you think the care you had was good enough for your friends and family.

The questionnaire is anonymous so it is ok to tell us what you think - whether it be good or bad!

There are no right or wrong answers and it is up to you whether you answer some or all of the questions - but the more information, the better! 

What you tell us will be compiled with other patients' questionnaires so we can get a good overall picture of how we are working. 

You can still complain if you are unhappy, the official complains procedure is to put your complaint in writing and to email it to bettsavenue@nhs.net for the attention of the Practice Manager. 

If there is something urgent, tell a member of staff straight away.
