Practice Assist: MAKING IT EASIER FOR YOU TO ACCESS HEALTHCARE

From 7th August, Pavilion Surgery will be working in partnership with Care UK to provide an extended telephone service in addition to the regular services we provide at our practice. This is to improve and increase access to medical support for our patients.

The Care UK clinicians will work in exactly the same way as our local clinicians. For your safety and to maintain the quality of consultations your medical records will be accessed by Care UK clinicians through a secure network and your information will not be used by any other third parties. Care UK is an organisation experienced in providing high quality NHS healthcare.

We know it can be difficult to get an appointment at your surgery.

Part of the reason for this is that many face-to-face appointments are used for problems that could be better dealt with over the telephone.

In response to this we are introducing a new telephone service to help you get the support you need.

Patients can ‘Opt-Out’ of the service by notifying the practice.

If you have any queries about the telephone service, please contact reception.

All the staff at Pavilion Surgery would like to thank you for your co-operation during this time of change and we hope you find the new service beneficial.

SELF CERTIFICATION SICK NOTE FOR LESS THAN 7 DAYS

If you have been ill for 7 days or less you do not need to see a doctor for a sick note.

Some employers have their own self-certification forms or you can either download one from the website    https://www.gov.uk/government/publications/statutory-sick-pay-employees-statement-of-sickness-sc2 or pop into the surgery to collect one at reception. Please hand the form directly to your employer once you have completed it.
