New Patient Registration
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
We are no longer taking on new patients from outside our catchment area.
Patients who move out of the practice area will be asked to register with another GP nearer to their address.
In accordance with local guidelines all patients must provide two proofs of their identity when registering, e.g. birth certificate, marriage certificate, driving licence, passport, wage slip or NHS card. We will ask you to complete a registration form and a questionnaire to collect important medical data. Both forms can be downloaded below.
Some new patients will be asked to undertake a health check with a health care assistant.
Medical treatment is available from the date of registration. Please contact reception for further information.
Change Personal Details
If you change your name, address or telephone number please let our receptionists know or inform us of the change via emis access online.
If you move outside the practice area it will be necessary for you to find a doctor in your new area.
