Who can register at the Pavilion Surgery?

Anyone who lives within the practice boundary for Pavilion Surgery can register with us as a patient. To check if your residence falls within our boundary please use the boundary map tab, enter your postcode into the search box and it will check for you. Please do check this before submitting registration forms.

 

How can I get a registration form?

 

If you wish to register as a new patient with the Pavilion Surgery you can collect registration forms in person from the practice reception or download and print the forms bellow.

GMS-1 Form - this form must be filled out for both adult and child registrations.

What do I do with my registration form?

Once complete you bring your registration to the practice with;

  • One form of photo ID – driving licence, passport, identity card.
  • One proof of address – utility bill, council tax letter, bank letter

If you are a house bound patient wishing to register with the practice and are unable to return your registration forms physically to the practice, please contact main reception to discuss alternative options.
