New Patient Registration

The doctors welcome new patients who live within our practice area. Click here to see if your address falls within our area. To register with the practice you can complete forms online and bring them in or pick them up from the Reception Desk at the Health Centre .  Please also bring some identification with you (driving licence, passport or any utility bill), and if you are on any repeat medications bring a copy of your re-order prescription form from your previous practice.

If you have not completed one online, a new patient questionnaire will also be given to you along with a practice leaflet.

The practice area extends roughly to a nine mile radius around Elgin.  A detailed map of the practice boundary is also displayed in the reception area.

Online 'Pre-Registration' With The Practice

If you wish to register click on the link below to open the form. When you have completed all of the details print the form and sign it. You will still need to visit the surgery to hand in the form. You will have to bring some identification with you (driving licence, passport or any utility bill). If you are on any repeat medications bring a copy of your re-order prescription form from your previous practice. Please complete a separate form for each member of the family/household

We also need you to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this form, which  you must fill out and and print off . When you come to the surgery you will then be asked to sign this form to confirm that the details are correct. Again a seperate will need to be completed for each of the family/ household member

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
