New Patient Registration

The Practice covers Almondbury and the surrounding area.  A map displaying the practice boundary is available at our reception desk.

New patients are required to complete a family doctors registration form (GMS1) and a new patient questionnaire – these forms are available at our reception desk. 

Alternatively, you are able to complete a (GMS1) form on GOV.UK by clicking on the link below.  You will still need to complete a new patient questionnaire when handing in the (GMS1) form to our reception desk.

The completed registration forms should be returned to the reception desk after 1.00pm any weekday. Registration applications must also be accompanied by photographic evidence of identity and official proof of current address.

If you would like to register with Almondbury Surgery but live outside of our Practice Boundary please ask a member of our reception team for an “Out of Area – Choice of GP Practice Patient Information Leaflet”.
