Application for online users:

Firstly, please read the leaflet:

Patient Information Leaflet - It's Your Choice

so you fully understand the importance and implications of signing up.

Secondly, either complete a form available from Reception and hand it in to one of the Reception team or print one out from here

Application for Online Access

and take to Reception. YOU WILL NEED TO PRODUCE PHOTOGRAPHIC ID SUCH AS PASSPORT OR DRIVING LICENSE AT THIS POINT. We will not be able to activate an account for you without evidence of ID.

We will activate your account within 5 working days and produce your personal log-in details for you to take away - you must collect this letter in person and bring your ID with you again. 

Once you have your Patient Access Registration Information letter, please either click on the links on the Home Page of this site or go to https://patient.emisaccess.co.uk/Account/Login?

Follow the link to say that you have already registered.

Click on the green button to Register.

When prompted 'Have you visited your GO already and been given a PIN and an Access ID?' click Yes.

Using the information on your letter, enter the details and click 'Next'

The nex page will as you for your surname and date of birth.  You will then need to enter a password of your choice.  Click on 'Next'.

This will complete your registration and you will now be able to log on and book an appointment, request a repeat prescription etc.

PLEASE NOTE THAT YOU WILL NOT BE ABLE TO REGISTER FOR ANYONE OTHER THAN YOURSELF AT THIS TIME.

The following form allows a patient to give consent for our staff to discuss their medical records, book appointments, change appointments and collect prescriptions etc..

If this is something that you feel would benefit you then please fill in the form below and return it, either by hand or via email to sehccg.pinehillsurgery@nhs.net and a member of our team will update your records.
