Registering for our Online Services allows you to access a range of services and information via your computer, mobile or tablet.
You will need to complete an Online Services Form and show us photo ID. You will then be given an automatically generated username and password, which will allow you to login via the below website links.
Parents/Guardians/Carers may register children, but you will need to show us their birth certificate.
If you don't have any ID, please contact Reception for advice.
Download our Online Services Access Form here. You can print it and send it to the surgery by hand or post, or email it to warleymedicalcentre@nhs.net with a clear photograph or scan of the requested documents.
