Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Information Needed For Registration

 

Proof of Address:

Tenancy Agreement. If you don’t have one you must get a signed letter from the landlord stating you are their permanent resident.

 

Current bill (e.g. gas, water, electric, bank statement) with your name & registration address.

Must be dated within the last 2-3 months.

 

 

Proof of Identity:

 

Valid Photo ID Passport Or Drivers Licence – with registering address on.

 

If you have had a previous GP in the UK please provide:       NHS Medical Card.

 

If you have never had a GP in the UK please provide: Passport with valid Visa giving you permission to stay in the UK. If you do not have this you must provide letters/documents form the Home Office giving you permission to stay in the UK.

 

 

 

Children under 16 years:

MUST bring their original full Birth Certificate & Child Health Record Book – needed for immunisation details.

 

 

You must bring all relevant information to the surgery for the receptionists to verify before you can be given an appointment to register. Please bring your information between 10:30am - 4:00 pm Mondays - Fridays only.

 

Once the receptionist has verified your information as being valid for acceptance onto the GP List you will be given an appointment for Registration where you will be seen by the Practice Nurse & GP.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
