Social Prescribing
The practice now pleased to announce that we are able to provide an additional resource to help patients who may find themselves in the situation whereby they require support. Below is a link for patient to self refer if they feel they could benefit from this service.
Please note the following criteria before completing the referral form:
Over 18’s -
- Assistance with finances, debt, benefits, employment and higher education
- Low grade mental health issues eg sleep issues, anxiety, low mood.
- Social isolation and keen to connect with others or community groups
- Physical inactivity
- Over/under weight and wanting support to tackle this
- Issues following COVID eg isolation, bereavement, loss of income/employment, loss of support networks.
- Relationship concerns
- Specific situations that may benefit from extra community support eg veterans, mild learning difficulties, new mothers, homelessness, refuge
Children 8 1 18 - School issues – bullying, isolation and friendship problems, pressure with schoolwork
- Family – parental separation, family illness, young carers, bereavement, children witnessing domestic abuse/drug + alcohol abuse
- Employment, financial, higher education, careers advice
- Physical inactivity
- Weight management
- Smoking, Alcohol, Drug use
- Relationship advice – sexual health, relationship breakdown
- Homeless support
- Refugees/asylum seekers
- Learning difficulties
- Mental health – mild anxiety, low mood, sleep disturbance

Family Planning
Contraception care is provided by all the doctors during surgery hours. Our practice nurses can also provide advice on family planning. Appointments can be made for coil cap fittings and checks.
Flu Vaccination
An influenza vaccination is particularly recommended for patients with heart, lung or kidney disease, diabetes, carers and residents of nursing and rest homes and those over 65years of age. This may be subject to change due to national health authority guidance.
Child Health Surveillance
A check by a Doctor is offered at 8 weeks for baby and Mum’s post-natal. Development Assessment is offered by the Health Visitor based at Chase Hospital.
Asthma Patients
It is recommended that patients with asthma are seen annually. This allows the Practice Nurse with specialist training to asses you and helps you develop a self-management plan according to your needs
Diabetic Patients
It is recommended that patients with diabetes attend a 6-12monthly review with our Senior Practice Nurse, who specialises in diabetic care. Patients are sent an appointment for a review which includes a blood test appointment two weeks prior to the annual review. If you feel you are overdue for a check-up, please contact reception for an appointment.
Health Checks
It is recommended that patients with a history of heart problems should be reviewed annually. A blood test two weeks prior to this appointment with the Practice Nurse will be needed. This will allow the Practice Nurse to monitor your condition, blood pressure, medication and to assist in a self-management plan
Non NHS Services
Certain services provided by your doctor are not covered by the NHS and you will be asked to pay a fee, eg travel vaccination including yellow fever, pre-employment medicals, insurance claims, holiday certificates, private medicals, private certificates, fitness to drive medicals etc. We also act as examiners for several life insurance companies. Should we be asked to examine you for their purposes the company will be responsible for the fee.
Fees chargeable are in line with current British Medical Association recommendations. A list of the fees is posted at reception.
