Afghanistan- Support for military Veterans

We are conscious that some patients registered here at the Grange may be affected by current events in Afghanistan. Our Practice is accredited by the Royal College of General Practitioners as one which specialises in the care of military veterans and their families. All the team have been trained in the care of military veterans and their families.

Temporary Closure of the Keldregate Branch- Come to our Online Drop-In Session

NHS England Data Opt-Out (ACTION NEEDED BY 30th OF SEPTEMBER)

General Practice Data for Planning and Research (GDPR) Opt-Out Guidance

The data held in the GP medical records of patients is used every day to support health and care planning and research in England, helping to find better treatments and improve patient outcomes for everyone. NHS Digital has developed a new way to collect this data, called the General Practice Data for Planning and Research data collection.

You can choose to Opt-out of this data collection. You need to do this by the 30th of September (this has changed from the previous deadline of the 23rd of June). Choosing to opt-out will not affect your care.

 

What is data used for?

Data collected will be used to support a wide variety of research and analysis to help run and improve health and care services including:

  • Informing and developing health and social care policy
  • Planning and commissioning health and care services
  • Taking steps to protect the public (including managing and monitoring the coronavirus pandemic)
  • Enabling healthcare and scientific research
  •  

What data will NHS Digital collect?

This data will be shared from 1 July 2021. Data may be shared from the GP medical records about:

  • any living patient registered at a GP practice in England when the collection started - this includes children and adults
  • any patient who died after 1 July 2021, and was previously registered at a GP practice in England when the data collection started

NHS Digital will not collect patients’ names or addresses. Any other data that could directly identify patients (such as NHS Number, date of birth, full postcode) is replaced with unique codes which are produced by de-identification software before the data is shared with NHS Digital.

NHS Digital Will Collect:

  • data about diagnoses, symptoms, observations, test results, medications, allergies, immunisations, referrals, recalls and appointments, including information about physical, mental and sexual health
  • data on sex, ethnicity and sexual orientation
  • data about staff who have treated patients

NHS Digital Does not collect:

  • name and address (except for postcode, protected in a unique coded form)
  • written notes (free text), such as the details of conversations with doctors and nurses
  • images, letters and documents  
  • coded data that is not needed due to its age - for example medication, referral and appointment data that is over 10 years old
  • coded data that GPs are not permitted to share by law - for example certain codes about IVF treatment, and certain information about gender re-assignment

Carers Week 2021 (7th - 13th June)

Are you a carer? Do you know someone who is?

Why not get involved in carers week 2021! The theme this year is 'Making Carers Visible and Valued'. This is about raising awareness of the challenges which unpaid carers face, as well as highlighting the valuable contribution they make to their families and our society. There are around 6.5 million carers across the UK who deserve this recognition!

To celebrate carers week, Carers Count have put on different events for carers ranging from fun to informative, including stress management, movie club and a benefit awareness session.

Keeping Patient's Hearts Healthy with West Yorkshire and Harrogate Healthy Hearts

Heart and circulatory disease is the biggest cause of death and disability in England, with 1 death every 4 minutes.

Supporting Unpaid Carers and People with a Personal Health Budget (PHB)

There is currently a FREE training programme available to support unpaid carers and people with a PHB now and beyond Covid-19. The training is led by an experienced clinical tutor via a digital platform. The training lasts 45 minutes and offers support with:

• Keeping the care space safe

• Managing Linen

• Disposing of personnel waste

• Basic hygiene for visitors

• Handwashing

• Basic Personal Protection Equipment gloves, aprons and masks

COVID Vaccine Update (4)

Further update regarding the Covid Vaccine 09/02/21

All of our patients who are over 70 and extremely clinically vulnerable have now been invited for their Covid vaccination, as per government guidance.

Vaccinations for housebound patients are well underway and we aim to have vaccinated all housebound patients by the 15th of February.

Please don't worry if you haven't had your vaccination yet, we will continue to invite ALL eligible patients until everyone has either had their vaccination or declined it.

Please don't contact the practice regarding your vaccination unless you have received an invitation but cannot book online.

The vaccination site that we are currently inviting patients to is Cathedral House, St Thomas' Road, HD1 3LG


COVID-19 Vaccine Update (3)

Patients over 80 will be invited shortly if they have not been invited already. There are 2 ways that patients in this group may attend for their vaccine:

  • You may receive a letter from the NHS inviting you to attend a national vaccination centre. Currently the closest of these at the moment is in Manchester, information on how to attend will be in your letter.

  • If you cannot travel to a national vaccination centre location offered in this letter you do not need to do anything and should wait to be contacted again. Patients are also being invited to attend a smaller vaccination site in Huddersfield, and patients will continue to be contacted as more appointments become available. 

  • Housebound patients will be contacted by the practice as soon as supplies of the AstraZeneca (Oxford) vaccine become available.

Please note we do not have specific dates as guidance is frequently being updated by NHS England. This page will be updated when we have further information. Once again, we kindly ask patients NOT to contact the surgery regarding the Covid vaccination. Doing so may prevent people that need medical help from getting through to us.


COVID-19 Vaccine Update (2)

The first phase of the NHS Covid-19 vaccination programme has started.

When it is the right time people will receive an invitation to come forward. For most people this will be a letter, either from their GP or the national NHS.This letter will include all the information you will need to book appointments, including your NHS number.

Please do not contact the NHS to get an appointment until you get this letter. Information on the vaccine is available on the NHS.UK website.

 


COVID-19 Vaccine Update

The Pfizer BioNTech vaccine has now been approved for use and the NHS will be starting to offer this to priority groups as soon as supplies arrive.

When will a vaccine be available?

The first deliveries of the vaccine should be during w/c 7 December and the NHS is ready to start offering them as soon as they arrive to those who need them most.

Who will get a vaccine?

The vaccine will be offered to those at greatest risk from Covid19 first, starting with people living in care homes, people over 80 years old and frontline health and social care workers. This is based on the guidance from the Joint Committee for Vaccinations and Immunisations which is available here. As more supplies of the vaccine or alternative vaccines become available it will be rolled out in phases to people aged between 80 and 50 and those who are clinically extremely vulnerable. People will be invited for a vaccine when it is their turn so they do not need to contact their practice or the NHS.

Where can I get a vaccine?

To start with, vaccines will only be available at hospital ‘hubs’ where the Pfizer vaccine can be stored safely. However, arrangements are also being put in place to offer vaccines from a number of different locations as more supplies and different vaccines become available. These include local vaccine services provided by GPs and pharmacists and new dedicated vaccination centres to make sure that everyone who needs a vaccine is able to get one.

We will share further information with you as it becomes available. In the meantime, there are three things people can do to help:

· Please don’t contact the NHS to seek a vaccine - we will contact you when it’s the right time to you to have yours

· Please act on your invite when it comes, and make sure you attend your appointments when you arrange them;

· Please continue to abide by all the social distancing and hand hygiene guidance, which will still save lives.


IMPORTANT - IT IS MANDATORY TO WEAR A FACE COVERING WHEN ATTENDING THE SURGERY.


Polite Notice

The Keldregate branch was deemed to be clinically unsafe to operate from during the covid pandemic. As such, we have taken a decision in accordance with our local CCG to temporarily close this site.

We have increased our opening hours at The Grange site at Spaines Road (HD2 2QA), and we are pleased to offer GP services from 8am – 8pm Monday – Thursday, and 8am – 6:30pm at The Grange.

Our patients enjoy the benefits of increased access to GP services at the Grange, and we encourage all our patients to use our digital triage (eConsult) package – available via our website. If you are unable to access the eConsult, please call us on 01484 428282 and we will be happy to help.

Calls to and from the surgery are recorded

Calls to and from the surgery are recorded and processed in accordance with the General Data Protection Regulation 2016 and the Data Protection Act 2018, calls are recorded for monitoring, training and dispute resolution purposes.

The purpose of call recording is to provide an exact record of the call which will:

  • Protect the interests of both parties;
  • Help improve Practice performance and best practice;
  • Help protect Practice staff from abusive or nuisance calls;
  • Establish the facts in the event of a complaint either by a patient or a member of staff and so assist in resolving it;
  • Establish the facts and assist in the resolution of any medico-legal claims made against the practice or it clinicians;

A call recording may also be used as evidence in the event that an employee’s telephone conduct is deemed unacceptable. In this situation the recording will be made available to the employee’s manager, to be investigated as per the Practice Disciplinary Policy

How to book an evening or weekend appointment at the HUB:

You can also book an evening or weekend appointment by ringing the practice on: 01484 428282. If you need to book an appointment when the practice is closed then you can ring the free NHS 111 phone line out of normal working hours and they will book an appointment for you if appropriate.


Extended Access evening and weekend appointments now available

Patients can now see a local GP, Physio and Phlebotomist for routine appointments on weekday evenings and at weekends. Pre-bookable and on-the-day appointments are available between 6.30pm and 8.30pm each weekday, with additional slots available on Saturdays and Sundays between 8.45am and 5.15pm.

Appointments for all three services are available for any Greater Huddersfield patient in the Primary Care Centre hub at Huddersfield Royal Infirmary and some physio appointments are also available in Holmfirth and Armitage Bridge.


As of 1st November 2019 we will be operating an emergency access line between 12.30-2pm everyday. The practice will remain open and our reception team will be happy to deal with any patients who present at reception. General telephone access will be available everyday from 8.30am till 12.30pm and 2pm till 6pm.


Changes to our services

We are sorry to inform you that we are having to make changes to the way we practice. Over the last few years we have had a reduction in clinical staff due to doctors and nurse practitioners leaving and retiring. Despite much effort we have struggled to fill those posts and it means that we are having to do things differently.

We now have a pharmacy team who will deal with all medication queries, do medication reviews and manage some minor ailments. Our reception staff are actively signposting patients to more appropriate services as sometimes the problem can be managed differently and by other people/professionals.

Due to the reduction in staff we are having to change how we do home visits - we will continue to visit all registered bedbound patients if required and those receiving terminal palliative care. However, all others will be phoned first to see if the visit is required or if it can be managed differently. If you go to the shops, hairdressers or are elderly then you do not automatically qualify for a visit.

Please note all our clinical staff are highly qualified with one or more degrees, diplomas, specialist training and will try to help you with your health problems.


Active Signposting

Active Signposting is one of the ten NHS England High Impact Actions that practices can take to free up GP time. Active Signposting is designed to connect patients more directly with the most appropriate source of help or advice; this may include services in the community as well as within the Practice.

All of the reception/admin team here at The Grange Group Practice have undergone both online and face-to-face training commissioned by our Clinical Commissioning Group (Greater Huddersfield CCG) in order to go live with Active Signposting within our Practice on 19th March 2018.Active Signposting will offer the patient ‘choice’ to access the service that is most appropriate for them. Reception staff will not ‘triage’ patients; instead they will be able to connect the patient more directly with the most appropriate professional or service in our area. Reception staff will not make clinical decisions but when a patient presents with symptoms that could be dealt with by a pharmacist, optician or a social prescribing service they will confidently offer these alternatives.

This new initiative gives patients more control over their own care because it offers them the choice of a service that better suits their health and wellbeing needs. Active Signposting also improves access for patients who do need to see a GP, as patients who do not necessarily need to see a GP are directed to a more appropriate service.

Active Signposting aims to change the assumption that GPs are the first point of care for patients – this benefits patients by improving outcomes and creating a better experience, which in turn improves job satisfaction for our hardworking and dedicated reception/admin staff.

Our services were inspected by the Care Quality Commission (CQC) on 19th August 2015. We are very pleased to announce we were given a "GOOD" rating.
