Patient Confidentiality
All Clinicians and members of staff at St Isan Road Surgery have a duty to respect and maintain patient confidentiality. Furthermore, subject to service provision requirements, St Isan Road Surgery contracts with third party healthcare individuals and organisations such as locum doctors, nurses, clinical agencies and pharmacists who have a professional duty of confidentiality.

If you want to make an 'Individuals Rights Request' please use our form, supplied by our Data Protection Officer - Individuals Rights Request Form and return this to our Individual Rights Administrator, Mrs Sara Clark, either via email Sara.Clark@wales.nhs.uk or hand it into reception or post addressed to the administrator at St Isan Road Surgery, 46 St Isan Road, Heath, Cardiff, CF14 4UU.
