Please telephone 02920 612333, or visit reception for details.  You will be asked to complete a 'new patient health questionnaire' and to make an appointment with a Health Care Support Worker at your earliest convenience, to complete the registration process. 

Please let us know if you have any disabilities that may cause you problems accessing our services i.e. hearing or sight impairments or any communication problems.  Let our team of receptionists know and they will guide you through the registration process and make sure you don't experience any barriers in accessing health care advice and treatment.

*we will not keep copies of ID or proof of address as part of your medical record, we only need sight of these documents.

A Patient Guide to GP Services

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups. Open the leaflets in one of the following languages

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Patient Confidentiality

All Clinicians and members of staff at St Isan Road Surgery have a duty to respect and maintain patient confidentiality. Furthermore, subject to service provision requirements, St Isan Road Surgery contracts with third party healthcare individuals and organisations such as locum doctors, nurses, clinical agencies and pharmacists who have a professional duty of confidentiality.

No personal information will be passed on to any other sources without your prior consent. 

What are your rights?

The General Data Protection Regulation (GDPR) 2016 determines how your personal data is processed and advises on how to keep this data safe. It also stipulates your rights when is comes to processing you data dependent on the purpose and legal basis used. Please read our Privacy Statement for further information. 

If you want to make an 'Individuals Rights Request' please use our form, supplied by our Data Protection Officer - Individuals Rights Request Form and return this to our Individual Rights Administrator, Mrs Sara Clark, either via email Sara.Clark@wales.nhs.uk or hand it into reception or post addressed to the administrator at St Isan Road Surgery, 46 St Isan Road, Heath, Cardiff, CF14 4UU.

Welcome Students

To register as a patient with us you can either download and complete the GMS form here, which you can email to us at admin.w97053@wales.nhs.uk or you can register with us via GP Registration Form (campusdoctor.co.uk)

If you have any chronic conditions, or any health care needs that require ongoing medication please let us know when you register.  We'll make sure you're able to keep ordering your medication as usual.

Book Appointments and Order Repeat Medication Online

You only need to contact us by phone for anything urgent or if you need to see a GP the same day.  Everything else can be done online by registering with our appointment booking and repeat prescribing website My Health Online MHOL form.  The site makes visible all available appointments so you are able to select the doctor you want to see at a time that suits you.  You will also receive a reminder of the appointment the day before and you can cancel the appointment through the site as well.
