New Patient Registration

Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number.

How Do I Register With The Practice?

You can now register with the practice by completing the form below (please click the link to open the word document.)

Please then email it back to Chgp.prg@dorsetgp.nhs.uk

If you do not have access to emails or a way of downloading form, you may wish to collect a form from practice and bring it back to us in person. 

Temporary residents are registered in a different way, please contact reception on 01202 772540.

New Patients

In the same document as the registration form is a health and medical history questionnaire to fill in and you can ask for a health check by the Nurse if requested. This is important as it allows us to get to know you better and collect important information on past-medical health, current problems and medication. It is also an excellent time to ensure that all immunisations, contraceptive requirements, cervical smears and blood pressure measurements are up to date. There can sometimes be a delay in obtaining your records from your previous Doctor and therefore the new patient information is vital to your new Doctor.

Foreign Visitors

Please ask at reception for details on how to register as a temporary resident.  Please be aware that charges may apply if you are referred to secondary care.
