If you would like to know more about our Online Access services please click on the link below or ask at the Surgery for information. To use these services you will need to register by completing the form below and submitting it to the Practice together with proof of identity (eg Passport or Driving Licence)                                              

Our online services can be access by completing the following forms.

Ordering Repeat Prescriptions/Booking an Appointment:To order repeat prescriptions and book an appointment online you will need to complete 'Access Online Registration - pt 1' (see below)

Access On Line  Registration Form - pt.1

Viewing your Medical Record: To access your medical record you will need to complete 'Access Medical Records - pt 2' (see below) as well as pt 1 above

Online Patient Access Services: - Safety and Security

Once we have processed your request we will issue you with personal login details, individual to you.  It is your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it then you should change your password immediately.
