Access to your Medical Record
Under the The General Data Protection Regulations 2018 (GDPR) and the Data Processing Act 2018 you have the right to access your personal data.
This is sometimes called a Subject Access Request (SAR).
You can do this verbally to anybody in the practice, electronically, or in writing. It is helpful if you use this form: Request for Copy of Record
We will respond within one month. We may need to ask you for documents to prove your identity.

National Data Opt Out (NDOO)
Also on 25th May the NHS Digital launches the National Data Opt Out. You can choose whether to allow NHS Digital to use your confidential data for Research and Planning.
Confidential information means information that identifies you and says something about your health, care or treatment.
