Your Electronic Records
Electronic records are held in all the places where a patient receives healthcare. In the past these places have only been able to share information from your records by letter, email or phone, which can take time and slow down your treatment.
The secure Electronic Computer system used by Siam Surgery, and other NHS services to hold your records, is called SystmOne. With your consent, this system has the ability to allow healthcare professionals who are caring for you to share your medical information with each other electronically.
Your Decision
You can now choose how your medical information is shared:
Sharing OUT:
This controls whether we can share your electronic record with other NHS Care Services where you are treated.
Sharing IN:
This controls whether we can view information recorded by other NHS Care Services where you have received treatment.
We now need all of our patients to make this decision, so we can record your preferences in our system. When you attend Siam Surgery we will ask you to complete a form to let us know your preferences. However, if you are not a frequent attender at the Surgery, we would be pleased if you could download the form below and send it to us to let us know your preferences.
