Our local population is growing fast and there is a huge demand for appointments, both with the doctors and the nurses. We have been looking at ways to improve appointment availability and have found many appointments are wasted each week because the patient is not booked in with an appropriate clinician. For example:

  • Not all doctors undertake joint injections
  • Not all doctors undertake minor surgery
  • Not all doctors undertake coil fitting
  • Some doctors have more specialised knowledge in particular areas
  • Not all nurses undertake travel vaccinations
  • Not all nurses undertake cervical screening
  • Some nurses have more specialised knowledge in particular areas

If you are booked with a doctor or nurse who is unable to deal with the problem for which you are attending, it is not only a wasted appointment for the doctor or nurse, but also a wasted visit for you as you may be asked to re book.

There are some procedures which cannot be undertaken if there is not a second nurse or a doctor on the premises, for example immunisations.

It helps us to know the reason for routine appointments where equipment needs to be prepared, for example, a particular injection may need to be collected in advance from a central storage area or may even be out of stock and need to be ordered.

We are aware some patients object to being asked the reason for their appointment, our receptionists have received comments such as ‘What has it got to do with you?’ ‘Why do you want to know are you a doctor?’

Please be assured, if your visit is for a medical problem which you do not wish to discuss with the receptionist, we quite understand and are more than happy for you to tell us you would rather not give the reason.

Statement of Intent

THE ELECTRONIC PATIENT RECORDS:

SIAM SURGERY STATEMENT OF INTENT

 

New contractual requirements came into force on 1st April 2014. These requirements state that all GP Practices must make available by the end of September 2014, a statement of intent in relation to the following IT developments:

 

1. Patients Online Access to their GP Record

2. GP to GP Record Transfers

3. Summary Care Record (SCR)

 

This is Siam Surgery’s statement.

 

Patient Online Access:

 

NHS England requires practices to offer their patients online access to make appointments, request repeat prescriptions, and view information regarding their allergies and adverse reactions, and to promote this facility. This should be completed, or the practice should have plans in place to achieve this by 31st of March 2015.

 

We currently offer the facility for booking and cancelling appointments and ordering repeat prescriptions on-line. If you do not already have a user name and password for this system – please follow the link on the website for an application form, or ask at reception for more details.

 

GP to GP Record Transfers:

 

NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers with a new practice.

 

It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England. It can take your paper records up to two weeks to reach your new surgery.

 

With GP to GP record transfers your electronic record is transferred to your new practice much sooner. Siam Surgery are already active with GP2GP electronic transfers and we are able to send and receive patient records via this system providing the receiving practice has this system.

 

Summary Care Record (SCR):

 

NHS England require practices to enable successful automated uploads of any changes to patient’s summary information, at least on a daily basis to the summary care record (SCR) or have published plans in place to achieve this by 31st of March 2015.

 

Having your Summary Care Record (SCR) available, will help anyone treating you who does not have access to your full medical record. They will be able to see information about any medication you may be taking and any drugs that you have a recorded allergy or sensitivity to.

The core SCR record contains medications, allergies and adverse reactions and is uploaded on the basis of implied patient consent. Additional information can only be uploaded with explicit patient consent and should only be information required to support patient care in an emergency or urgent situation.

 

The SCR should only be viewed in emergency or urgent care settings with the explicit consent of the patient unless the patient is unable to give their consent (for example a patient is unconscious).

 

If you do not want your medical records to be available in this way, you will need to let us know, by completing an ‘Opt Out’ form, so that we can update your record.

 

We confirm that your SCR will be uploaded before the end of March 2015, and will be automatically updated on at least a daily basis to ensure that your information is as up to date as possible.
