Named GP: As from June 2015 we are obliged to inform patients that they have a “named GP”. This has no bearing at all on which GP you currently see and / or wish to see in the future.

Due to the current surge in patient demand for appointments, alongside a significant increase in requests for advice or appointments via email, with immediate effect Clinical team will no longer respond to patient queries received via email

If you do not feel that you need an appointment but would like some clinical advice, or want to send a photo of a rash or skin lesion, please use our online triage system. Your query will be reviewed by a clinician within 48 hours

Any queries sent direct to clinician emails will not be actioned

Anyone, of any age, can access our Online Services. If you are a Parent, and the Patient is under 11, you do not need consent..

Due to the coronavirus pandemic, the practice is receiving an unprecedented amount of telephone queries; therefore, we would strongly advise you to access your online services, where you can request repeat prescriptions and view your medical records. 

  

How Do I Register for Online Services?

You can register  for either the NHS App, or Patient Access; or both. 

NHS App

If you are not registered for online services, please register via the NHS App
available on iOS and Android devices, such as Smartphones and Tablets

You are also able to access the NHS App through a web browser, for example, on your computer or laptop at https://www.nhsapp.service.nhs.uk/login 

Once you have registered, your account is approved by NHS, usually within 2 hours, Monday to Friday.

  

An alternative option available is to register for access to Patient Access instead, following the process below. 

 

Patient Access

Complete our Online Services Registration Form and return to the Practice along with the correct forms of identification (see below).

Alternatively, you can

  1. Complete the form electronically
  2. Email to STOCCG.MarpleCottageSupport@nhs.net 
  3. Accompanied with a photograph of each form of identification (as specified below),
  4. Aswell as a photograph of you holding the Photographic ID.

What Identification do I need?

0 – 10

A Parent is able to request Proxy access for a Child under the age of 11. Parents are required to provide one form from List A or B, one from List C, and Full Birth Certificate of the Child.

11 – 16

Patients aged 11 will need to provide both their Birth Certificate and Passport.

17 & over

You will need to provide one form of Identification from List A. 

Alternatively, one form from List B and one form from List C

Once you have requested access to your account, we aim to process requests within 5 working days, subject to verification of identification, and examination of sensitive / third party information. 

 

 

How Do I Register/Login?

You will need to wait until you receive your Registration Letter from the Practice. This could be via email or printed by our Receptionist. In order to login and access your account, you will need to register your details and email. See below.

Video Consultations

The practice offers Video Consultations with the Doctor and our Asthma Nurse.

This service is accessible via Computer, Tablet and Smartphone. 

To use the Video Consultation System, you will need to register for our Online Services. 

To Register, complete our Online Services Registration Form and bring to the Practice along with the correct forms of identification. 

Once you have successfully registered, you will then be able to book a Video Consultation with a GP or our Asthma Nurse. 

Once you have booked your Video Consultation, you will need to login to the online appointment system five minutes before you appointment and join.

Online Access to Medical Records Policy

At Marple Cottage Surgery we encourage patients to view their medical records via the online medical viewer service and provide online access to information such as:

  •  Allergies
  • Immunisations
  • Medication
  • Test results
  • Consultations*
  • Hospital letters*
  • Procedures
  • Problems/diagnoses

3.3 Restrictions to access

There are occasions when a GP may firmly believe that it is not appropriate to share all the information contained in the individual’s record, particularly if there is potential for such information to cause harm or distress to individuals, or when the record has information relating to a third party.

 

Patients may request paper copies of health records and, regardless of the preferred method of access, patients and authorised third parties must initially complete a DSAR form. However, patients may request access to their health records informally[1] any such requests should be annotated within the individual’s health record by the clinician dealing with the patient. 

  • Access to records of child under 11yr to be granted to parent / guardian with “parental responsibility” which is defined, under the Children’s Act 1989, as
  • Online access automatically defaults to the child once they reach age 16.
  • To have discussion with parents in a situation where ongoing access by parents would be in child's best interests e.g. long term condition / learning disabilities
  • Child will need to demonstrate competence to access own records from age 11 onwards (GP may wish to see / speak to child to assess competency)
  • Child proxy access to parents up to age 11 whereby automatically switched off at 11th birthday (as per RCGP guidance)
    • having the child live with the person with parental responsibility, or having a say in where the child lives;
    • Or if the child is not living with her/him, having a personal relationship and regular contact with the child / controlling, guiding and directing the child’s upbringing.
