New Patient Registration

Patients living within the practice area are welcome to register, and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill and passport to the surgery.

You will also need to complete a registration form (GMS1) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team.

Medical treatment is available from the date of registration.  Please contact reception for further information.

It is important that we have your NHS number when registering as this saves time locating your medical history. We will need your first language and whether an interpreter is required for appointments and whether they should be male or female.

Changes to Personal Details

Please notify the practice of any changes in relevant personal details such as change of name, address, telephone number.
