Patient Registration
When you register at the surgery you will be asked to complete a GMS1 form. You will also be given a health questionnaire, which must be completed as fully as possible and returned with your GMS1 form. You can also register with the practice on-line by following the links below.
The practice boundary covers Hale Barns and parts of Hale and Timperley. Please ask our administration team for further details.
We can only register patients that live within the practice boundary.
If you move to an address that falls outside of our boundary you will need to register with another practice. In order to be fair & equitable we are unable to make exceptions on an individual patient basis.
Out of Area Registrations
The practice has opted not to participate in the Patient Choice Scheme which allows GP practices to register patients who live outside the practice boundary, without any obligation to provide home visits or out of hours services when the patient is unable to attend their registered practice.
New Registrations
If you are interested in registering, please fill out the following registration forms IN FULL. Incomplete forms will causes a delay to your registration being processed.
Please ensure that you fill in ALL the details requested as failure to do so may delay your registration.

- GMS1 Purple form
- Health Questionnaire
NOTE: If you are a parent registering your child, or are under 18 years of age please complete the health questionnaire but exclude the alcohol monitoring.
Please bring in any immunisation history you may have.
ALL FORMS MUST BE COMPLETED FOR REGISTRATION
Once registered patients have the right to express a preference of practitioner but your named accountable GP will be Dr M Patel.
