You will be required to complete two forms available at the reception desk or download and print below for each individual wanting to register.  

The practice will refuse an application if you reside outside the practice area or if the list is closed. You will not be refused for any other reason.

We are currently accepting registrations from patients that live within the practice boundaries. We are not registering patients who live outside of the practice boundary.

You may print off a registration form HERE, fill it out and bring it in with you on your first visit to the practice. Please also print and complete the relevant questionnaire (adult/child)

When you apply to register at the practice we will ask you for two form's of ID (one photographic and one residential). Anyone aged 16 or above would be expected to attend the surgery with their own ID. Parents or guardians are able to register on behalf of children below the age of 16. We WILL NOT refuse your application to register at the practice if you do not have the relevant ID as long as your reside within the practice boundary as per our Safer Surgeries Declaration - Safer Surgeries Declaration

Named Accountable GP

When registering at this practice all patients are allocated to one of the 2 GP partners. The GP partner you are allocated to is assigned as your named accountable GP. This is for the purpose of our clinical records and to allocate overall responsibility of your care at this practice. The process of allocating to a GP partner has always been used here; some practices choose to register patients as a group practice rather than to an individual GP.

Ellenbrook Medical Centre is a 2 partner practice and even though you have been allocated a GP you can choose to see whichever GP you wish routinely. If you ask to see a GP urgently you will see whichever GP is on call for that session.

Allocation of named accountable GP is slightly different for patients aged 75yrs+ and those patients on a tailored health programme. These patients can expect to receive an individual letter outlining their allocated GP.
