New Patient Registration

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of photographic ID, such as a passport or photo driving licence as well as proof of address such as a recent utility bill. Please also bring along your medical card if you have one.

Please note that we only accept patients who reside in the BL1 postcode. Any existing patients who currently live outside the BL1 postcode are able to stay registered but will be asked to change GP's if they then move to another address outside the BL1 postcode. If you move outside of the BL1 postcode you will need to change your GP practice.

You will need to complete a registration form (GMS1) and a medical questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration.  Please contact reception for further information.
