New Patient Registration
We welcome new patients from within our practice boundary area. At the time of registration we will require
- Proof of identity & address.
- Eligibility to stay in UK.
Every new patient will need to have a new registration health check providing information regarding medical history & medication etc.
Medical treatment is available is only available after a health check with the nurse. Please contact reception for further information.
This practice does not register patients immediately, but only following a health check with nurse.
