All new patients are required to provide two acceptable forms of identity and residence in the UK prior to registration, one of which must be photographic (i.e. passport or driving licence). Other acceptable forms include council tax documents, rent book, utility bills, television licence, original marriage/birth certificate (not recently issued), flight tickets to the UK (single journey) and bills for shipping of personal belongings to the UK. 

Entitlement to NHS care in Scotland is generally based on residency status not nationality. In all cases the responsibility for providing such evidence lies with the individual.

You will also need to complete a registration form and you get the option to make an appointment with one of the nurses for a new patient health check. You will also be asked to provide a specimen of urine for routine testing. 

Registration is at the discretion of the Surgery. You will be registered with the Surgery and not with an individual doctor,  but you can request to see a particular doctor if you wish. If you are unsure if you live within the practice boundary a map is available at reception .

Medical treatment is available from the date of registration.  Please contact reception for further information.
