New Patient Registration

We are pleased to be accepting new patients. You can register online by clicking here or please read the information below and complete the forms at the bottom and drop them into reception. These are also available within reception if you would prefer. If you would like online access to your records or to order medication online, we will also require Photo ID and proof of address at the point of registration.

If you live outside our practice area you may still register with the surgery, but we cannot provide home visits for you.

Once registered with the practice

You can then register for on-line services which will allow you to

  • Book appointments
  • Order repeat prescriptions
  • Have access to test results

This can be done during our opening hours at reception.

You may want to make an appointment to meet the doctor and go through any health problems and medications, the doctor can also check that any tests required are up to date. 

If you have long term medical conditions that requires an annual review we will we contact you when the review is due.

Medical treatment is available from the date of registration. Please contact reception for further information.

Please remember to bring with you Photo ID and proof of address when registering.  You can also bring with you a completed Registration Form & Patient Questionnaire or complete these in reception.
