This form is a request to book an appointment with a Doctor ONLY.

This form is for routine appointments ONLY.

You need to put a reason for the apointment to ensure we are giving you the correct service.

If you feel you have an urgent medical problem or need a same day appointment please ring the surgery 0161 277 6939.

• You can book appointments via this website,  on the NHS App, through Patient Access and via the telephone. 

• Please note that the telephones are very busy between 8am and 9am each morning. 

• On the day appointments are available from 8am on the NHS App.

Each day we offer urgent appointments for conditions that will not wait until the next routine appointment.

These appointments will not necessarily be with the Doctor of your choice or a Doctor of the sex of your choice.

Each day we also offer book on the day appointments and these can be booked each morning when you ring at 8:00am.

Booking appointments in advance is available but not encouraged by the practice due to the increase of non-attended appointments.

Please be aware reception staff will be required to obtain a reason for any appointment to ensure the most appropriate service is accessed.

Cancellations & Non-Attendance - 

We have a high number of appointments that are not cancelled by the patients which costs the NHS a lot of money. Please can you inform the practice as soon as possible if you are unable to attend your appointment on 0161 277 6939. We have a policy of three DNA's and then you are removed from the practice list and will not be allowed to register again. Please speak to reception if you have any questions.
