Travel Service

The practice provides a travel service to our permanently registered patients.

Our travel appointments are limited so a travel questionnaire must be completed and handed in to reception at least 6 weeks before you travel.

Some vaccinations require more than one injection to provide adequate protection so 2nd & 3rd appointments maybe required.

We cannot provide travel advice without receipt of a completed travel questionnaire.

Travel questionnaires can be downloaded by clicking on the link below or collected from reception.

If you miss a travel appointment we may not be able to re-schedule before your departure date.

Details of our travel charges are clearly displayed on the reception desk and can be viewed by clicking on the Travel Service Price List tab at the top of this page. Please take the time to check the travel charges prior to handing in your questionnaire.

You are advised to contact the surgery 48 hours after submitting a travel questionnaire to be advised of which vaccinations are recommended for your destination.

Full payment by card, cheque (payable to Dr M Patel) or cash will be required prior to your consultation.
