Changes To The Way You Order Your Repeat Prescription
As from 2nd September 2019 Pharmacies will not be able to order medicines on your behalf. You will need to order your repeat prescription directly from your GP practice either online or by using the white tear off part of your repeat prescription to the practice, identifying which medicines you require.
Nurse Clinics
Our Practice nursing staff are Claire Bradbury Practice Nurse and Susan Lightfoot Nursing Assistant.
Patients please be aware when booking an appointment to see a specific nurse that you have booked into the appropriate clinic appointment slot. For example ear syringing/smears can only be done by our Practice Nurse Claire Bradbury.
If in any doubt please ring the surgery and the reception team will book your appointment in to the correct clinic.
C Card is now available at the Haven Surgery
C Card is a condom distribution scheme that provides registered young people 13-24 years with a C Card which entitles them to free condoms. It also gives young people access to sexual health information, advice and support.
This initiative allows you to pick up free condoms from over 200 outlets throughout County Durham and Darlington once you have completed some straightforward registration details. The C Card is a plastic card the same size as a credit card.
If you would like a C Card you can now attend the surgery to register. If you already have a C Card you can attend The Haven Surgery or any outlet to pick up free condoms.

Volunteer Transport Service
Help to Health is a volunteer driver service which became available on the 15th June 2017, for patients of all ages who struggle to use public transport to attend a planned healthcare appointment. The Help to Health team will assess each patient for suitability when they register with the service.
Help to Health will transport eligible patients to planned primary and secondary care health appointments. A map of the area that will be covered can be found on the help to health website www.helptohealth.org.uk.
Journeys will need to be booked at least 48 hours in advance and a volunteer driver being available at the required date and time. The transport will incur a charge of 45p per mile travelled and the payment of any car parking fees or expenses incurred. People that can, claim back their travel costs through the Healthcare Travel Costs Scheme.

Therefore, there should be no Home Visit requests to the GP Surgery because a patient does not have access to transport. In the time that it takes to do 1 Home Visit, a GP could be seeing several more patients in the Surgery with better access to investigations.
Well Being For Life - A Huge Success for our Patients
Well Being for Life sessions at The Haven Surgery with huge success.
The practice has been proactively reviewing pre-diabetic patients with an Hba1c score between 42 - 47 who are at higher risk of developing Diabetes and Heart Disease. This cohort of patients are offered face to face appointments with the Health Trainers at the Practice. The Health Trainers then support our patients to make a small difference to their lifestyles, to greatly improve health outcomes later in life and reduce the risks of developing Diabetes and Heart Disease.
Patients receive an individual assessment with support offered for nutrition, weight management, physical activity and generally improving their lifestyle. Patients are then regularly reviewed at the surgery.
