New Patient Registration

consultation1The doctors welcome new patients who live within our practice area. 

Please ask at reception for a registration form. An appointment will be offered to see a Health Care Assistant and we will require completion of a medical questionnaire which is confidential. This provides us with up to date detailed information about your health whilst we are waiting for your medical records to arrive.

We will also carry out a new patient medical which includes a blood pressure check and ensures we have all the relevant information completed on your registration forms. You may specify a Doctor that you wish to be treated by, however the practice registers its patients on to a practice list not an individual GP list.

 You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
