New Patient Registration

We welcome new patients who live within our practice area and out of area as per new arrangements introduced from January 2015. The new arrangements mean GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.

If you wish to register at Highcroft Surgery you will need to provide proof of address. Photo ID is preferred e.g. Passport/Driving Licence but we can accept with either a bank statement or utility bill if you do not have a form of photo ID.

You will need to complete a GMS1 form along with a Registration form. The forms are available from Reception or you can download them here.

You can also complete the GMS1 form online by clicking here. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
