Permanent Resident New Patient Registration

If you would like to join the patient list at Palatine Group Practice, please attend the practice reception at any time - no appointment is necessary - along with a valid (i.e. in date) photographic ID and a proof of address.  As proof of address, we will accept a recent (within 3 months) utility bill, a bank statement or similar.  We cannot accept a mobile phone bill.  Examples of photo ID would be a passport, a driving licence, or similar.  Forms will be issued on attendance and we would be grateful if these could be completed as promptly and thoroughly as possible.  Your application should be then be processed within a week.

If your name has recently changed, e.g. if you have just got married, but you have not yet got your ID changed, please bring your marriage certificate with you.
