WEEK COMMENCING 25th MARCH -FRIDAY 5TH APRIL THE PRACTICE WILL HAVE REDUCDED GP AVAILABILITY.  FOR ROUTINE ISSUES PLEASE CONTACT THE PRACTICE WEEK COMMENCING 8TH APRIL.  THANK YOU FOR YOUR CO-OPERATION. 

THE PRACTICE OFFERS ROUTINE FACE TO FACE APPOINTMENTS UP TO 48 HOURS IN ADVANCE. PLEASE CALL THE PRACTICE FROM 8.30AM MONDAY TO FRIDAY. PLEASE NOTE YOU MAY BE ASKED TO CALL BACK ONCE WE HAVE REACHED CAPACITY.

 WE WOULD KINDLY ASK PATIENTS WHO HAVE SYMPTOMS OF A RESPIRTORY ILLNESS TO WEAR A MASK WHEN ATTENDING THE HEALTH CENTER.

OVER THE COUNTER MEDICINE

Processing these prescriptions can cost 20 times the price of buying identical medication at your local pharmacy or supermarket

THE PRACTICE WOULD LIKE TO ASK  PATIENT'S WHO REQUIRE MEDICATION FOR HAYFEVER AND ALLERGIES TO PURCHASE THESE AT YOUR LOCAL PHARMACY OR SUPERMARKET. THANK YOU FOR YOUR CO-OPERATION.

 

**ZERO TOLERANCE POLICY**

Please note the practice operates a zero tolerance policy. In recent months we have noticed a significant increase in abuse towards our staff. At Cityview Medical we treat all our patients with dignity and respect and we expect all our staff to be treated with respect. We will not tolerate any abusive language, threatening behaviour or aggression towards any member of staff. Any violation of this policy will result in removal from the practice list.

PIP APPLICATIONS

As a Practice we no longer complete requests from DLA / Attendance allowance / PIP forms OR for supporting evidence.

We strongly believe that to get an accurate assessment a face to face consultation is vital.  This needs to be carried out by the agency making the decision about your application, not by your GP.
