New Patient Registration

Patients living within the practice area are welcome to register and our reception staff  will be happy to assist. For guidance the practice covers the area bordered by the River Hodder in the east, River Ribble in the south, the M6 to the west, and the River Brock up to Bleasdale in the north.

We regret that we cannot accept new patients living outside these boundaries.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

You may print off a registration form and a health questionnaire, fill these out and bring in with you on your first visit to the practice.

If you are on any repeat medication it is advisable to make sure you have enough from your previous GP to last a couple of weeks as we will need to register you and you will also need to speak with a Doctor before anymore prescription can be given.

Medical treatment is available from the date of registration.  Please contact reception for further information.
