New Patient Registration

New patients are welcome, both private and medical card holders. When you come in please ask for a registration form.

Once you are registered you have access to Lukedoc and Dubdoc, our excellent out-of-hours service.

Registration is free.

When you register, we will ask you to sign a letter to release your medical records from your previous GP, which we will then arrange to be transferred.

If you have a current Medical Card or Doctors visit card or are using an E11 card you need to have your current card details with you to register.

Completed forms should be brought to reception to be signed by our doctors.
