New Patient Registration

New patients will be asked to complete a health questionnaire. Please ensure that you give us full details as it may take some time for your medical records to arrive from your previous doctor. You will be offered a new patient check with the practice nurse.

Patients have a right to express a preference of practitioner however this may be dependent on availability. We do not discriminate on the grounds of age, sex, marital status, pregnancy, race, ethnicity, disability, sexual orientation, religion or belief. 

Documentation Required for Registration

If you are new to the area and wish to register with our practice please bring along the following documentation:

  • Proof of identification (photographic) and your passport. 
    You will also be asked to show proof of address, for example a utility bill, (although it is accepted that if you have just moved to the area you may not have this at the time of initial registration). If you are visiting from overseas other documentation may be required, the Reception Team will advise on the specific requirements.

When registering you will also be asked to complete our New Patient Registration forms as this allows the doctor to familiarise him/herself with your medical history and healthcare needs.
