ANTICIPATORY CARE PLAN ALERT

Anticipatory care encourages people to make positive choices about what they should do themselves, and from whom they should seek support, in the event of a flare up or deterioration in their condition, or in the event of a carer crisis.

It is a one page document which provides specific information on what the preferred plan of action is.  These are usually completed for patients with conditions such as COPD, dementia, people in nursing and residential care homes or for patients with cancer.  The information is shared with NHS24 and hospitals so other Doctors are aware of how you would like to be treated when we are closed.
