NHS vaccinations and when to have them 

Vaccines are the most effective way to prevent many infectious diseases and it is important that vaccines are given on time for the best protection but if you or your child has missed a vaccination then please contact the surgery to make a catch up appointment.

Primary Care Networks

 Primary Care Networks

Primary care networks include groups of general practices working together, and in partnership with community, mental health, social care, pharmacy, hospital and voluntary services in their local area, to offer more personalised, coordinated health and social care to the people living in their area.

There are 43 PCNs in Kent and Medway. Networks are aligned to a health and care partnership - this supports them to work more closely with other partners in the area.

The networks were established in 2019 and have already been working together to provide more services outside the standard surgery opening times. They worked together through the pandemic to maintain primary care services and a large part of the Covid-19 vaccination programme was co-ordinated by PCNs.

Green Porch Medical Centre is a member of Sittingbourne PCN which has 6 member practices including Chestnuts Surgery, Grovehurst Surgery, Meads Medical Practice, Memorial Medical Centre and The Medic Care Surgery.

The population of Sittingbourne as at 1.1.23 is 64,767.
