Station House Surgery Closure Dates

The Surgery (including Dispensary) will be closed for mandatory training from 1pm on:

Thursday 13th January 2022

Thursday 10th February 2022

The practice will re-open as normal at 8.00am the following day

Please check you have sufficient medication supplied whilst we are closed

If you have Coronavirus symptoms or are living with someone who has, you are advised to self isolate for 7 or 14 days respectively. Please visit https://www.nhs.uk/conditions/coronavirus-covid-19/self-isolation-advice/ to obtain an isolation notice for your employer. You do not need to contact the practice.

 

 

 

Access to Medical Records

BACKGROUND

In accordance with the General Data Protection Regulation, patients have the right to access the data and information held by Station House Surgery. Data subjects have a right to receive: 

    • Confirmation that their data is being processed
    • Access to their personal data.
    • Access to any other supplementary information held about them


OPTIONS FOR ACCESS

Online Access

This service enables you to make & cancel appointments, order prescriptions and view information in your electronic health record.

If you wish to register for this service please complete an application form (which you can download below).  Take this to Reception along with proof of ID (Passport or Driving License and evidence of home address, such as a utility bill, bank statement or council tax bill).  Reception will then be able to generate your on-line access registration letter.

Requesting Copies of Your Record

You can request to be provided with a copy of your health record.  There is no fee for this service.

Please submit a Data Subject Access Request (DSAR) form (downloadable below).  You can either return this to the practice or email it to mbccg.stationhousesurgery@nhs.net.

We can provide you with the DSAR form if you would prefer. Let us know by speaking to Reception or emailing us at mbccg.stationhousesurgery@nhs.net

Time frame

Once the DSAR form is submitted, Station House Surgery will aim to process the request within 21 days; however, this may not always be possible. The maximum time permitted to process DSARs is one calendar month.

Exemptions

There may be occasions when the data controller will withhold information kept in the health record, particularly if the disclosure of such information is likely to cause undue stress or harm to you or any other person.                  

How do I apply to access my medical records online?

You must read the Information leaflet below, complete the application form and present photographic identification and proof of address.

You can also request Proxy access to your children's medical records - please speak to us if this is something you would like.
