  • type 2 diabetes

  • asthma or COPD

  • heart conditions, such as angina, a previous heart attack, heart failure or atrial fibrillation

  • a previous stroke or TIA

WHAT HAPPENS?

  • Blood tests with our nurse, about a week before your review

Missed a review? You haven’t lost your place.
Phone 0402 39841 and say you are due your chronic disease review. Reception will book your blood test and review together.

ASTHMA PATIENTS

Before your asthma review, please fill in our short questionnaire so we can make the most of your appointment. If your symptoms are getting worse, book an appointment now rather than waiting for your review.
