When do I need a Fit Note? (these are no longer called sick notes)

Key points

If you are off work for less than 8 days, you do NOT need a fit note signed by a GP.  Your employer may ask you to confirm you have been ill by completing a self certification (SC2) form which you can find here.

If you are off work for 8 days or more, your employer may ask for a fit note. If you are getting care from the hospital, request the fit note from your hospital doctor.  Otherwise, please request the fit note from the GP surgery by completing an online eConsult form.  We well get back to you by the end of the next working day.  If a doctor agrees that a fit note is needed it will be issued within 2 working days of your request.

If you are self isolating because you have flu like symptoms or have a household member affected,  and need a note for work, please follow this link.

Please do not ask the practice for a sick note for Covid related issues.  These requests will be denied at this time because of our workload.
