A full and detailed itinerary must be included on the travel questionnaire. We ask that you also include a day time contact number so you can be contacted if we have a query with your form. Travel questionnaires can be collected from the surgery or accessed on-line by following the link below.
You are advised to contact the surgery 48 hours after submitting your travel questionnaire to be advised of what vaccinations are recommended for your destination.
Full payment by card, cheque (payable to Dr Patel) or cash will be required prior to your consultation.
