Named Accountable GP for All Patients

Every patient has been allocated a named GP who is responsible for your overall care at the practice. If you would like to know who your named GP is, please contact reception and they will be happy to inform you.

If you wish to change the name of your accountable GP, please let us know and we will make every  reasonable effort to accommodate this request.

Having a named accountable GP does not restrict your entitlement to see any doctor in the practice.

Day Lewis Travel Vaccine Clinic

  1. Call MASTA on 0330 1004106
  2. A nurse will register your details for you. State that you would like to use travel clinic at Day Lewis Pharmacy
  3. The nurse will then book an appointment with our pharmacist to give you your vaccination and travel medication

We keep a limited supply of the following travel vaccines at the surgery and are happy to administer these, free of charge, if appropriate and booked far enough in advance. We require a copy of a travel risk assessment when booking an appointment:

  • Hepatitis A (Adult and Junior)
  • Typhoid
  • Tetanus/Diptheria/Polio
