Online Services Records Access - Your Choice

If you wish you can now use the internet to book appointments with a GP, request repeat prescriptions for medication you take regularly and look at your allergies and immunisations on lines. You can still use the telephone or call in to the surgery for any of these services as well It's your choice.

Being able to see your record online might help you to manage your medical conditions. It also means that you can access it from anywhere in the world should you require medical treatment whilst on holiday. If you decide not to join or wish to withdraw, this is your choice and practice staff will continue to treat you in the same way as before. This decision will not affect the quality of your care.

To access online services you will be given login details and you will need to think of a password that is unique to you. This will ensure that only you are able to access your record - unless you choose to share your details with a family member or carer.

At the Valentine Medical Centre surgery in Blackley, the doctors and nurses take pride in offering the highest standard of patient centred healthcare. We run many clinics for chronic disease care and offer a wide variety of other on-site medical services including; minor surgery, midwife-led antenatal clinics, childhood vaccinations, on site clinical pharmacist, counselling and alcohol/drug services.

Prescription Requests

Please note all prescription requests will take 48 hours surgery cannot take prescription requests over the telephone.

Prescription requests can be posted through the letterbox, ordered on-line or ordered through your pharmacy.

We only accept telephone requests from patients who fall into the following categories:

Elderly (Aged 65+)
Housebound

New service

A website is now live to help find your NHS number. This service is for anyone living in England who has forgotten or does not know their NHS number. You can also use this service on behalf of someone else where the name, date of birth and registered home postcode is known. You can opt for the number to be sent to you by text, email or letter.
