Dear Patient

This is to let you know about the Patient Participation Group – These are the important items we have discussed: NHS Choices Website, GP Practice Survey (MORI) & Friends and Family Test.

  • Did Not Attend Surgery Appointments, we have a large number of people not attending every month.
  • Impact of the new build in Selly Oak on our Surgery.
  • Family and Friends - 2 Replies and all would extremely likely recommend the Surgery.

You can now access your medical records online. To do this we need the following:

Picture ID and a Bank Statement.

COVID-19

Please do not attend the surgery unless you have been given permission to do so. If you need to request your prescription you will able to do so through the Birmingham and Solihull Health App. We will take request over the phone where necessary. 

If you have any concerns about Covid-19 you can visit gov.uk or 111online website, alternatively you can call 111 for further assistance.   

 

 

 


ECONSULT -  IT'S HERE JANUARY 2024 

Please click the link below to access our new on line econsult booking form.   This is the easiest way to access appointments, suppor, clinical and non clinical advice, request medication or fit notes.  If you are unable to complete the form please contact the GP reception team who will assist you.  

Please note the form is not currently available on the NHS App.

SIGN UP FOR YOUR NHS APP 

The NHS App allows you to access a range of NHS services. You can download the NHS App on your phone or tablet. You can also access the same services in a web browser by using clicking the image below

You must be aged 13 or over to use the NHS App. You also need to be registered with a GP surgery in England or the Isle of Man. Find out more about the NHS app by clicking the image below .

COVID 19 - GENERAL PRACTICE - JUNE 2023 

You should not visit the practice if you have tested positive for Covid 19, you must advice our team if you have symptoms of cough, sneezing, sore throat, sickness or diarrhoea.   

At Selly Oak Health Centre we treat vulnerable patients and were therefore require patients to use the available hand sanitser and all patients are required to wear a mask for their consultation.   

We ask that you help protect the NHS and our staff by adhering to the above rules when attending any appointments at the practice.

Repeat Prescriptions Online

You can use your NHS app to request repeat prescriptions.  If you need any help accessing your Online account please contact our admin team who can guide you.

New to the area and would like to join our practice? We would be happy to help you. Fill in our registration forms and come see us. Please note we will ask for Photographic ID and proof of address to help us process your registration.    Find out more

PRIVATE LETTERS 

If you require a private letter/report or access to health Records you will need to complete and sign the appropriate forms ( please ask at reception).   We will only accept request from the patient or appropriate or on behalf of the patient with the appropriate paperwork/authority/consent from patient.   There is also a charge for any letters and we anticipate to process your request within 2-3 weeks.
