From 1st November 2022 patients will be granted access to their prospective medical records when you sign up for GP online services.

 

You will be able to see consultations, lab results, documents, and immunisations. To register for online access, you will need to provide ID documents. You should provide one document from group one and one document from group 2. If you cannot provide ID documents and you are known to a member of our clinical team they may be able to verify your identity by vouching. Please ask at reception.

Being able to see your record online may help you to manage your medical conditions. It also means that you can access it from anywhere in the world should you require medical treatment on holiday.

If you decide not to join or withdraw from online services this will not affect your care.

It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately. If you cannot do this for some reason we recommend that you contact the practice and ask us to remove online access until you are able reset your password.

Some things to consider before you apply for online access to your record, there are some other things to consider:

 

There may be something you have forgotten about in your record that you may find upsetting.

If you have access to test results or letters, you may see something that you find upsetting. This may occur before you have spoken to your doctor or whilst the surgery is closed, and you cannot contact us.

Its up to you whether you share your information with others – perhaps family members or carers.  Its your choice, but also your responsibility to keep the information safe and secure.

If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.
