**ZERO TOLERANCE POLICY**

Please note the practice operates a zero tolerance policy. In recent months we have noticed a significant increase in abuse towards our staff. At Cityview Medical we treat all our patients with dignity and respect and we expect all our staff to be treated with respect. We will not tolerate any abusive language, threatening behaviour or aggression towards any member of staff. Any violation of this policy will result in removal from the practice list.

PIP APPLICATIONS

As a Practice we no longer complete requests from DLA / Attendance allowance / PIP forms OR for supporting evidence.

We strongly believe that to get an accurate assessment a face to face consultation is vital.  This needs to be carried out by the agency making the decision about your application, not by your GP.

ravel Vaccinationsglobal air travel

If you require any vaccinations relating to foreign travel you need to contact a local travel clinic in the first instance to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

The travel vaccine clinic will then advise you which vaccinations are required.

Some travel vaccines are ordered on a private prescription and these incur a charge.  This is because not all travel vaccines are included in the services provided by the NHS.

Any private vaccines can be then administered by the travel clinic.

The travel clinic will then advise you of which free NHS vaccines, if any, you require.  You can then contact the practice and we will arrange for the Practice Nurse to prescribe these and arrange for administration.

IT IS IMPORTANT TO ATTEND THE TRAVEL CLINIC AS EARLY AS POSSIBLE, AT LEAST 6 WEEKS BEFORE YOU TRAVEL AND YOU NEED TO BE VACCINATED AT LEAST 4 WEEKS BEFORE YOU TRAVEL TO ALLOW THE VACCINES TO WORK.
