As of 1st APRIL 2016, patients who move to an address outside the practice area of 8 miles, will be asked to move to a GP practice nearer their new address.
If you are moving within the 8 mile radius, please inform us of your address, postcode and the names of everyone who is moving to the new address so that we can update our records.
