Becky’s role is to provide patients with information, advice and guidance around the non medical support available for them, as well as with help to source social care at home. She is able to connect and refer patients to social care agencies, practical help at home, bereavement and counselling services, community groups, as well as many other services and social activities. She helps elderly patients, carers, patients with disabilities, isolated and vulnerable adults, and any patient looking for ways to stay independent and improve their daily life. |
