Repeat Prescription Request Form
Notice for patients - Prescription Minimum Intervals
Please note that we are increasingly using Minimum Intervals on our prescriptions - this means that the computer will not be able to process requests made too early ie more than 10 days before the prescription is due.
The reasons we are doing this are -
- for patient safety
- to reduce medicines waste
If you happen to request a prescription too early (ie outwith the 10 days limit) and this cannot be processed you will be notified via a red slip in with your reorder form from your chosen pharmacy.
The item will then need to be reordered.
Thank you for your co-operation with this change of system which is to help keep patients safe with regards to medicines.
