Hospital Clinic Letters

If you have recently attended hospital and you have been started on a new medication please hand in your letter as soon as possible. Without this we cannot start the new medication.

If you are not given a letter from the hospital, the practice must wait for notification from the hospital to inform us of any changes. Without something in writing, we will be unable to action any changes.

Please note:

Unless stated in the hospital letter, these prescription changes are NOT classed as urgent so regular waiting times for prescriptions apply.

Discharge Letters

Discharge letters will normally be sent to us electronically. Any medication changes will be made after being reviewed by our pharmacotherapy team.  They may contact you to discuss the changes, were appropriate.

If you receive a blister pack from a pharmacy, we shall endeavour to update the pharmacy appropriately regarding any medication alterations.
